Endeavor Health – Peer services, health navigation, care coordination, behavioral health
Evergreen Health – Harm reduction, care coordination, housing, behavioral health
Fidelis Care – Insurance Enrollment Assistance
Highmark Blue Cross Blue Shield – Medicaid enrollment and renewal
BestSelf Behavioral Health – Behavioral health, crisis support, community services
Community Action Organization – HIV testing, linkage support, food & nutrition
Buffalo Adult and Teen Challenge– Impatient rehabilitation referrals
Spectrum Health – Safety and welfare assessments
Plymouth Crossroads-Residential supports.
Community Services
- Erie County Department
of Motor Vehicles. Driver’s license and ID renewals, registration and titling for vehicles, and permit testing. - Erie County Department
of Social Services. Assistance with Medicaid applications. - 716 Ministries. Job readiness and soft skills training.
- Job Corps. Career-training and education program for young adults aged 16-26 years old.
- Veterans One Stop Center. Housing, network connecting, financial counseling and peer support.
- WNY Mental Health Advocates. Social Security disability applications.
