Build Promise’s Healthcare and Community Services are available to all our neighbors in need, providing a wealth of vital resources and expertise through trusted partners.
Current Services as of June 1, 2026
Healthcare
- Care Coordination — for Medicaid patients with qualifying chronic health conditions. Find or coordinate with doctors/specialists, advocate for you, explain care plans, arrange rides, and link you to counseling or programs for substance use. Provided by Evergreen Health Jewish Family Services.
- Home Healthcare Screening For people with chronic conditions, determines if you are eligible to receive services at home.
- Harm Reduction Van — for anyone who uses drugs. Free overdose prevention, clean syringes, and judgment-free support. Provided by Evergreen Health.
- HIV Testing — Provided by Community Access Services.
Community Services
- Erie County Department
of Motor Vehicles
Driver’s license and ID renewals, registration and titling for vehicles, and permit testing. - Erie County Department
of Social Services
Assistance with Medicaid applications
